FAQs ON WORKFLOW, INFORMATION , AND ACCOMPANYING RESOURCES
1. TERMS AND CONDITIONS

Please review our company's Terms and Conditions. This information will also be provided at the end of the New Customer Form for your agreement prior to doing business with us. You will receive an email confirming your agreement upon completion of the form.

2. COMPANY POLICIES

Please review our Company Policies prior to working with us. Email policy inquiries to admin@trelfalabs.com.

3. SUPPLY ORDERS (If Applicable)

Common supplies your facility may need for routine testing include air plates, sponges, swabs, and sample bags.


Though in some cases, we may ask for reimbursements, Trelfa Labs does not purchase supplies and resell them for profit as a vendor. We are not considered a reseller by the state of Massachusetts and do not collect sales tax. 


Most supplies our lab provides, allocated for use in tests we perform on your behalf, are included in the cost. 


Our former Supply Order form has been updated to include sampling requests. Now you can use one convenient form, should you need both. This also benefits you in terms of booking time with our driver, as now you can give us multiple dates and times that would work for you instead of being forced to pick just one from our former booking system. The goals is to give our driver more flexibility to set up as-needed routes and keep logistics costs down.


Supplies can be ordered from us in advance via the Sampling & Supply Request Form. Please allow 1-2 weeks for delivery.


If your supply needs are time sensitive, we encourage you to purchase directly from our recommended vendors: Neogen and Weber Scientific.


Please verify that your supplies will be acceptable for the tests we are performing for you, if necessary. We are happy to recommend specific products for your needs. 

3A. SAMPLING, SUPPLY DELIVERY, & ENVIRONMENTAL SWABBING - LOCAL CUSTOMERS (If Applicable)

Determine how you will submit samples and/or receive supplies allocated for future analysis from the following options:


OPTION 1: SAMPLE DROP-OFFS / SUPPLY PICK-UPS (VIA CUSTOMER)


HOURS

  • 9 am – 5 pm, M - F, except Federal Holidays

NOTICE
  • Sample Drop-Off Requests: 4-hours
  • Supply Pick-Up Requests: 1-2 weeks

LOCATION
  • Close proximity to both I-95 and I-495
  • Plenty of free parking
  • A parking lot which accommodates semi-trucks, should you prefer to send a vehicle from your fleet
  • Easy ground floor access, with a vestibule stocked with Chain of Custody forms and room for multiple coolers, should the lab temporarily close

  • Minimum: $30 in sales

OPTION 2: SAMPLE PICK-UPS/ SUPPLY DROP-OFFS/ENVIRONMENTAL SWABBING (VIA LAB DRIVER)


SCHEDULING

Whenever possible our Logistics Team combines customer sample pickups with supply drop-off services on predetermined route schedules. See Lab Driver Travel Routes & Schedule to review your service area. Getting added to a prescheduled route will reduce your costs, which are primarily determined by distance from the lab. As needed requests will incur higher fees.

HOURS
  • Our lab's drive times do not exceed 90 minutes one-way from our lab in Salisbury, MA.
  • The last available pickup time for customers 1 hour or more in drive time from the lab is 3 pm, M - F, except Federal Holidays.
  • The last available pickup time for customers 15 minutes in drive time from the lab is 4 pm, M - F, except Federal Holidays.
  • If these parameters do not work for your company, contact us to request a special arrangement.

REQUIRED NOTICE
  • Sample Pick-Up Requests: 48-hours
  • Supply Drop-Off Requests: 1-2 weeks
  • Environmental Swabbing: 48-hours 
  • All Cancellations: 24-hours


PRICING

  • Minimum: $50 in sales
    • None of the charge below count toward the sales minimum.
  • Sample Upcharge (Rush and Holiday): $25 per sample (when available)
  • Supply Upcharge (Standard): Typically included in cost of the final analysis. See Quotation for details.
  • Supply Upcharge (Rush): Shipping, handling, and other upcharges for rush supply shipments from the vendor will be billed back for reimbursement.
  • Supply Upcharge (Allocation): Your acceptance of supplies allocated but not used for analysis we were expecting to perform on your behalf will be billed for reimbursement.
  • Lab Driver Upcharge (Standard): $5 - $50, depending upon your location and pick-up date. Round-trip driver fees cover the following applicable travel costs:
    • Labor: drive time, sampling, supply delivery
    • Operating Costs: mileage, tolls, parking
  • Lab Driver Upcharge (Delay): Every lab driver appointment window is 15-minutes. Delays due to samples not being ready will incur upcharges at the rate of $20/hour, billed in (rounded-up) 15-minute increments.
  • Lab Driver Upcharge (Missed Pickup): $50 for customer samples either unavailable or unacceptable for pick-up. Cancellations must be made at least 24-hours in advance to avoid this charge.
  • Environmental Swabbing Upcharge: $50/hour

REQUESTS

OPTION 1. Current customers on a prescheduled/monthly recurring lab driver route, proceed to the digital Chain of Custody-Order Form to prepare your samples, if applicable.

OPTION 2: Current Customers in need of:
  • as-needed sampling, and/or
  • supply requests, and/or
  • environmental swabbing services

3B. SAMPLING & SUPPLY DELIVERY - DISTANT CUSTOMERS (If Applicable)

  • Lab Driver Upcharge (Standard): $5 for drive time to a local courier drop-off location to ship supplies on your behalf.
  • Supply Bill Back: Reimbursement cost of shipping the supplies to you (typically via UPS).
4. BOARD OF HEALTH (Dairy/Frozen Desserts & Sushi Rice Customers Only)

Gather your local Board of Health contact(s) information and confirm monthly testing requirements with them. This information will be submitted on the New Customer Form. Our lab will email copies of your lab reports to your Board of Health contacts on your behalf.

5. ADDITIONAL AGREEMENTS (If Applicable)

In addition to our Terms and Conditions, there may be other legal documents to sign such as NDAs (Non-Disclosure Agreements) or long-term contracts that need to be agreed on before we can do business. You will have an opportunity to upload your company's documentation when submitting the New Customer Form. Please direct any questions to admin@trelfalabs.com

6. NEW CUSTOMER FORM

Please fill out our New Customer Form which captures your company's information and needs, and allows us to prepare quotations, and agreements (if applicable). You are under no obligation to do business with us by doing so. Terms and Conditions do not apply until you place your first order with us.

7. CORRESPONDENCE

9. PRICING

Depending upon your company's needs, we will either send you a standard price list including possible upcharges or prepare a quotation for your review.


Sales minimums on analytical testing is based on the total volume in sales per day, not each Chain of Custody/order.

10. PRE-PAYMENTS (If Applicable)

Pre-Payment Terms

  • Most projects (particularly consulting, training, and nutrition panel preparation) require pre-payment.
  • New customers must pre-pay for their first order, regardless of the products and/or services required. 
  • Shelf-Life Studies may be billed as the project progresses, with reports sent monthly, if set up on a 'pay-as-you-go' pre-paid payment authorization.
  • You may be required to pre-pay for lab supplies necessary to accommodate unusually high-volume testing

PAYMENT AUTHORIZATIONS
  • Trelfa Labs uses Zoho Payments for credit card and ACH/e-check processing.
  • By completing the following steps, you authorize Trelfa Labs to pay invoices on your behalf, as needed.

Our former Payment Authorization Form has been replaced with a more secure means of requesting your payment information. We will email you a link to a secure online interface to enter your information. Please do not hand deliver, email, text, or leave details of your payment information on voice mail.

ACH (E-check) & Credit Card Authorizations
  1. Email finance@relfalabs.com to request a payment authorization.
  2. Our accounting department will email you a secure link where you can add your account information.
  3. Follow the link and fill out your information. Our system will automatically update your account.
  4. Link expires in 7 days.

Correspondence
All billing related correspondence is emailed or available in the Customer Portal. In the event you want documentation mailed to you, a small upcharge will apply.

OUR PROCESS


1. Submit Your Request

Gone are the days of cluttered email boxes! Submit initial information for Jon Trelfa via projects@trelfalabs.com only. Every staff member on the Project Team, including Jon, has access to this shared mailbox. Emailing Jon, or other staff members, directly may result in mismanaged emails and delay your requests. Please include your project's name in the email's Subject Line.


2. Discuss Your Project with Jon

Someone on the Project Team will set up an appointment for you to discuss the details of your project with Jon.


3. Pre-Pay

All consulting, training, and other projects are pre-paid.  Rebecca will request permission to charge the payment method of your choice.


4. Project Set Up
Rebecca will set up your project in the Customer Portal.

5. Project Access
Rebecca will grant access to any company representatives you designate to the Customer Portal. (Keep in mind the Customer Portal contains all your company's sensitive information, including payments.)

6. Work Begins
The Customer Portal has a great, simple project management feature. You will see your Project, its associated tasks, status, and be able to add your own comments to those tasks. There is also a place for you to upload files for Jon.

Any alterations to your project must be made in writing either by emailing documentation to projects@trelfalabs.com. We do not accept alterations via phone calls, voice mails, or texts.

Request phone appointments with Jon, as needed, via projects@trelfalabs.com, with your project name in the email's Subject Line.

7. Project Completion
Once you Project is complete, Jon will upload the final documents to your Customer Portal for review. You will see the Project marked Closed. Your pre-payment will be applied to the accompanying invoice to mark it paid in full and emailed to you for your records.
12. SAMPLE PREPARATION - COMPLETE CHAIN OF CUSTODY
  • EVERY SAMPLE ORDER MUST BE ACCOMPANIED BY A CHAIN OF CUSTODY FORM.
  • Click to download our MANDATORY Chain of Custody-Order Form. Forms before 07.01.25 are no longer accepted. Not only does the new form allow for extra characters. It's completely updated, streamlined, composite-ready, and built to import.
  • For routine/pre-scheduled lab driver pick-ups and customer shipments or drop-offs simply use the Chain of Custody-Order Form.
  • For as needed/unscheduled lab driver pick-ups and/or supply drop-offs, use the Sample Pick-Up/Supply Drop-Off Form in addition to the Chain of Custody-Order Form.
  • Create 1 form per Purchase Order or Lab report.
  • Incomplete Form Upcharge: We are happy to help you fill out your Chain of Custody forms, if necessary. However, if no form is provided, or form lacks clear instructions, it will be sent to the back of the queue. If our technicians must spend additional time on your samples due to missing forms or unclear instructions, we will up-charge to your order. Charges vary based on time spent, not to exceed $25 per form.


Prepare 1 Chain of Custody/Order Form for each Lab Report and/or Purchase Order request in Excel, as follows:

  1. ACCT #: IMPORTANT! Must match your assigned customer account number exactly. Example: CUS-001
  2. ACCOUNT NAME: Enter as shown on your most recent invoice. Do not use a DBA.
  3. RELINQUISHED BY: Full name of your company representative responsible for relinquishing samples to the lab.
  4. PO#: Your company's purchase order (if applicable).
  5. DATE: Date samples are submitted to the lab.
  6. DELIVERY: Select from drop-down: carrier, drop-off, lab pickup
  7. ORDER NOTES: Brief notes specific to this order (if applicable).
  8. SEND RESULTS TO: Company representatives (your staff and/or 3rd party) who are to receive the results of this order.
  9. EMAIL: Email addresses for all company representatives who are to receive the results of this order.
  10. REPORT TYPE: Select from drop-down: Standard, Certificate of Analysis
  11. #: Sample number. Enter on applicable rows only.
  12. TEST: Select from drop-down. If compositing, select the test type you are compositing on the first row, then select test type Composite on the following row(s). 
  13. SAMPLE & LOT: Enter sample description & lot relevant to the test selected on the same row.
  14. TYPE: Select from drop-down: AP: air plate, FD: food, SP: sponge, SW: swab, WT: water, OT: other
  15. QTY:  Items are always billed by row. Value will always be 1. Do not edit.
  16. Ignore the Lab Use Only section at the bottom of the form.
  17. You can adjust the row heights and insert new rows as long as you copy down the formulas in Excel, but otherwise, do not modify the form or we will not be able to import your data.
  18. Save form, identifying your company name, form type, and submission date in the filename. Example: ABCMfg COC 101025.xlsx
  19. Email form to lab@trelfalabs.com.
  20. Print a copy to accompany your samples.
  21. Re-use your original Chain of Custody/Order form as needed, saving with a new date in the file name upon each submission.

Upon submission, we copy your form and add lab results and notations as needed, making every effort to further identify samples. Dairy/frozen dessert reports typically include the month and year tested in the PO# field. If re-tests are needed, we indicate it there as well. Example: Nov 2025 Retest

When final, we use AI to import your order into our LIMS system where it extracts the relevant data and enters it directly into your lab report. This saves our technicians a lot of time by not having to hand key every report. And, of course, it will eliminate typos.

Your designated company representatives will receive copies by email. A copy will also be recorded in the new customer portal where you can match the Lab Report/Sales Order Number to the Invoice.

REMEMBER! The more complete your form, the faster you get your results! 

Email alterations to your submitted Chain of Custody / Order form to lab@trelfalabs.com. We do not accept phone calls, voice mails, or texts. Our entire staff has access to this shared mailbox, so there is no need to copy other employees.

13A. SAMPLE PREPARATION - LABELING & PACKAGING

  1. Create a clean and sanitized environment by following food safety preparation and handling instructions.
    1. Food Manufacturers: follow your Good Manufacturing Practices (GMPs).
    2. Food Service Professionals: follow USDA guidelines.
  2. Determine the type of raw material or finished product to be sampled: liquid, dry, raw meat/poultry, fresh, frozen, etc.
  3. Take a temperature using a calibrated thermometer appropriate for the sample material.
  4. Record the following on the outside of each sample container. You may print labels using our standard 2″ x 4″ Sample Label Template (Avery 5163).
    1. Trelfa Labs’ Chain of Custody/Order Number – for internal lab use
    2. Name of the company shipping the sample.
    3. Name of the company responsible for payment, which may or may not be the same as the shipper.
    4. Sample Name/Description (try to limit to 30 characters)
    5. Sample Lot/Code
    6. Date and Time Sample was Pulled. We cannot be held responsible for packages in need of immediate attention upon arrival if we are not informed of the delivery date.
    7. Sample Type
    8. Temperature
    9. Sampler’s Initials
    10. Special Instructions
  5. Please make sure that your samples are:
    1. clean
    2. packaged properly/tightly sealed
    3. included with ice packs, if refrigeration is required; we do not recommend dry ice
    4. included with packing material to avoid damage en route
    5. properly identified so that they can be matched to your Chain of Custody/Order
    6. no threat to laboratory operations
    7. delivered within a time frame that matches the required testing method

13B-1. DRY GOODS (Spices, Pastas, etc.)

Required sample size minimum: 100 grams

  1. Whenever possible, materials and products to be sampled should remain in their original packaging. Trelfa Labs will accept individual samples weighing up to 25 lbs.
  2. When a completely sealed container of dry material cannot be easily used as a sample, use a sterile sampling device which allows sample to be taken from the material’s container without coming into contact with the sampler’s hands or other unsanitary surface.
  3. Transfer the sample from the sampling device into an appropriately sized seal-able, sterile polypropylene bag, such as a Whirl-Pak bag.
  4. Seal the bag by folding the top over a minimum of three times and bending the wire closures in a fashion that will secure the folds.
  5. Identify the sample within the container with the following information. You may use our printable Sample Label Template:
    1. Trelfa Labs, Inc. Chain of Custody/Order Number
    2. Name of the company shipping the sample.
    3. Name of the company responsible for payment, which may or may not be the same as the shipper.
    4. Sample Name
    5. Sample Lot/Code
    6. Date & Time Sample Pulled
    7. Sample Type
    8. Temperature
    9. Sampler’s Initials
    10. Special Instructions
  6. Store the sample in accordance with storage recommendations from the master packaging until it arrives at the laboratory.
  7. To prepare for delivery, follow these steps:
    1. Place sealed sample containers in an insulated box.
    2. Place packing materials, such as crumpled paper or foam packing “peanuts”, around the samples. Do not cover the tops of the samples with this packaging.
    3. Fill remaining head space with crumpled paper or other packing material.
    4. Place insulated top on the box.
    5. Seal box with packing tape.
    6. Identify box as “Lab Samples”. Include date and name of responsible person.
    7. Store the box in a refrigerated area until it is out for delivery.
    8. Ship samples same day or overnight to Trelfa Labs.

13B-2. FROZEN DAIRY DESSERTS (Hard or Soft Serve Ice Cream, Frozen Yogurt, Gelato)

Required sample size minimum: 100 grams

The number of samples necessary for a new company is usually dictated by your local health department. If they have not given you this guidance, we recommend starting out by testing one flavor from each machine per month to show your Board of Health that your machines are clean. When they are satisfied, you can request a scale back in the number of samples from your local Health Inspector.

  1. Clean the machine in accordance with manufacturer’s recommendations.
  2. Fill the machine with product.
  3. Allow freezing to occur.
  4. If using a disposable or cleanable cup:
    1. Dispense a small amount of product from the machine into the cup.
    2. DO NOT ALLOW CUP TO CONTACT THE MACHINE OR SWIPE THE CUP ACROSS THE DISPENSER. ALLOW PRODUCT TO FALL FREELY.
    3. Place lid on cup avoiding contact with inside.
  5. If using a Whirl-Pak bag:
    1. Remove the seal from the top of the bag by tearing from one side to the other.
    2. Pull on the white tabs to open the bag. DO NOT MAKE CONTACT WITH THE INNER SURFACE OF THE BAG.
    3. Rub the lower portion of the bag between your fingers and thumb to fully open the bag.
    4. Hold the bag so that the yellow tabs with the wires inside are resting on your thumb and index finger.
    5. Place the bag under the dessert dispenser and dispense product into the bag.
    6. Continue dispensing for a count of 4 seconds. This will provide an adequate sample.
    7. Close the bag by pulling on the yellow tabs with the wires.
    8. Roll the top of the bag down three times and fold the yellow tabs over to create a good closure.
  6. Identify the sample to be placed within the container or Whirl-Pak bag with the following information. You may use our printable Sample Label Template:
    1. Trelfa Labs, Inc. Chain of Custody/Order Number
    2. Name of the company shipping the sample.
    3. Name of the company responsible for payment, which may or may not be the same as the shipper.
    4. Sample Name
    5. Sample Lot/Code

    7. Identify box as “Lab Samples”. Include date and name of responsible person.
    8. Store the box in a refrigerated area until it is out for delivery.
    9. Ship samples same day or overnight to Trelfa Labs.

13B-3. LIQUIDS OTHER THAN WATER

Required sample size minimum: 100 ml

  1. Whenever possible, materials and products to be sampled should remain in their original packaging. Trelfa Labs will accept individual samples weighing up to 25 lbs.
  2. When a completely sealed container of liquid cannot be easily used as a sample, use a sterile sampling device which will allow the sample to be taken from the material’s container without coming into contact with the sampler’s hands or other unsanitary surface.
  3. Pour the sample from the sampling device into a sterile polypropylene container and seal immediately.
  4. Seal the top of the container using plastic tape applied in a counter-clockwise fashion.
  5. Identify the sample within the container with the following information. You may use our printable Sample Label Template.
  6. :

RAW MEATS & POULTRY

Required sample size minimum: 100 grams

  1. Whenever possible, fresh and frozen meat and poultry should be sent to the lab intact, in the container provided by the supplier. Trelfa labs will accept sample sizes up to 25 lbs.
  2. When completely sealed container of meat/poultry cannot be used as a sample (i.e. samples over 25 lbs or combo-sized containers), use a sterile sampling device which will allow sample to be taken from the material’s container without coming into contact with the sampler’s hands or other unsanitary surface.
    1. For ground or small-piece meat/poultry, turn sterile sampling bag inside out and take grab samples from several locations within the container. Then turn the bag back through itself with sample for sterile containment.
  3. Transfer sample from the sampling device into an appropriately sized seal-able, sterile polypropylene bag, such as a Whirl-Pak bag.
  4. Seal the bag by folding the top over a minimum of three times and bending the wire closures in a fashion that will secure the folds.
  5. Identify the sample within the container with the following information. You may use our printable Sample Label Template

  1. Use only the water collection bottles provided by the laboratory for bacterial testing.
  2. Open the bottle without making contact with the interior surface of the bottle.
  3. If testing from a faucet, allow water to run for 10 seconds before taking samples.
  4. Place the open bottle under the faucet or in the water and fill to the 100 ml line marked on the side of the bottle.
  5. Close and lock bottle with the tie attached to the side of the bottle.
  6. Identify the sample to be placed within the container with the following information. You may use our printable Sample Label Template:

You can request separate logins for each of your company representatives on the Customer Form. Please note: each representative will see the same information including sensitive payment information.

18. NET 30 PAYMENTS

Net 30 Terms
  • After your first order, we will most likely switch you to Net 30 terms for routine lab analysis only.


Credit Lines

  • Most customers start out with a low credit-line, which can increase over time, based on good payment history.
  • Your company will be asked to provide professional references for high credit line requests.

Credit Hold
  • Overdue accounts are placed on Credit Hold with all lab reports, consulting, and training projects withheld until your account is made current.
  • Net 30 terms may be revoked if your account is put on Credit Hold for becoming overdue.
